‍‍Our Role:

‍At Create Outcomes, you will be part of a team who are navigating the complex world of psychotherapy together. We have a strong focus on your professional growth and development. When you begin your position with us, you will create a Life-Work Map. This map will provide both a short-term and a long-term vision for your desired path as a clinician. Your supervisor/consultant will use your map during weekly individual sessions to ensure that you have the support that is necessary to reach your goals and help your clients reach theirs. Our clinical team brings a breadth of experience and expertise. In our monthly group case consultations, you will have the opportunity to utilize the support of our team.

You will be trained in the C.R.E.A.T.E Outcomes Model which integrates psychodynamic principles and cognitive behavioral interventions. For example, your supervisor/consultant may ask you to examine your own bias and the client’s unique development when making treatment goals. This model includes a collaborative treatment plan that we refer to as the Life Map. Our model provides an approach to assessing, engaging, and treating clients. You can learn more about the Create Outcomes model here. We have the structure and support in place when you are working with clients who are high risk or in crisis.

Our education department offers continuing education credits for clinical trainings that are available to staff and to clinicians outside of the organization. Training costs are waived for those within Create Outcomes.

‍Your Role:

‍You will be responsible for providing therapy for adults and groups. We require that you attend weekly individual supervision/consultation and that you are motivated to learn. Our group consultations are held on Friday mornings (1-2x per month). We highly value clinicians who aspire to become supervisors and clinical directors at Create Outcomes.

This position begins as a part-time role and will grow into a full-time position. We require that you have some evening hours available to accommodate client schedules. We ask that you offer both in-person and virtual sessions depending on the needs of the client. For any client being seen in the office, we operate in compliance with COVID-19 safety guidelines and procedures.

Position Purpose​

The therapist in the Behavioral Health and Wellness Clinic provides competent, patient-centered, best-practice and evidence-based behavioral health and wellness services under the supervision of the Director of Behavioral Health and Wellness. The primary purpose of this position is to enhance the Clinic’s capacity to provide psychotherapy including individual, couple, and family services in the clinic that are high quality and patient centered. To this end, the Therapist will conduct intake, assessment, treatment planning and therapy throughout the care and discharge processes, including follow-up care.

However, the Therapist is also expected to participate in the delivery of other behavioral health and wellness services that are guided by educational, psycho-educational, and psychotherapeutic perspectives and at various levels of practice.  Furthermore, the Therapist must have an appreciation for the “business behind the business” and be willing to engage in various administrative tasks and duties that support and enhance departmental functioning.

The Therapist reports directly to the Director of Behavioral Health and Wellness.  The Therapist is responsible for service.

 

Essential Functions

  • Participate in the delivery of a variety of behavioral health and wellness services including educational, psycho-educational and psychotherapeutic interventions at various levels of practice, giving primacy to individual, couple, and family psychotherapy.  This includes on-call/after-hours care. This may require obtaining additional training, specialized expertise, certification, and/or licensure.
  • Participate in administrative tasks related to a variety of behavioral health and wellness services (e.g., program and policy development/maintenance, marketing, research activities) including assisting with the financial/billing tasks of providing patient care.
  • Keep abreast of the changing developments in the treatment of HIV and mental health/substance use conditions.
  • The Therapist (in conjunction with the Director of Behavioral Health and Wellness) ensures that all his/her clinic mental health patients receive quality assessments and an accurate diagnosis including initial screening and appropriate referral for substance use disorders, facilitates admission to treatment facilities and provide follow-up interventions as necessary.
  • Prepare, formulate and evaluate an appropriate treatment plan for each assigned patient, based on assessment and diagnosis, focusing on improving each patients’ total quality of life.
  • Manage each treatment plan in consultation with appropriate persons and service providers.
  • Act as a liason between patients and their collateral contacts.
  • Prepare accurate and timely reports, correspondence and patient record documentation.
  • Ensure that all necessary information, documents, and evaluations are in the official case record.
  • Develop knowledge of and maintain regular communication with the emerging specialized service providers and other circles of care/treatment providers (agency and community resources) to facilitate patient access and to continually coordinate care.
  • Actively participate in the Behavioral Health and Wellness Clinic staff meetings, individual and group supervision sessions, and continuing education events/trainings.
  • Regularly report to the Director of the Behavioral Health and Wellness Clinic on all assigned responsibilities.

Responsibilities

Direct Clinical Services & Case Management
• Provide clinical screenings, intake assessments, diagnoses, and treatment for adults with severe and persistent mental illness using evidence-based approaches.
• Develop and implement individualized treatment plans in collaboration with clients, grounded in their goals and strengths.
• Carry a manageable caseload of 35–40 adults, maintaining high-quality, person centered care.
• Offer individual and group therapy that supports recovery, self-advocacy, and community connection.

Coordination & Collaboration

Crisis Response
• Identify urgent mental health conditions and intervene using trauma-informed deescalation strategies.
• Make appropriate referrals to emergency or psychiatric treatment facilities when necessary.
Supervision & Program Development
• Provide clinical supervision to master’s level intern(s) and participate in intern recruitment, training, and evaluation.
• Collaborate with the Clinic Team Lead to support program goals and development.
Documentation & Compliance

HIRING SALARY RANGE: $59,740 - $74,740 

Incentives and Stipends:

  • Any special services provider with a current National Board Certification will receive an additional stipend of $2500 (pro-rated based on FTE)
  • $25,000 Fully Licensed Stipend: candidates who posses an active LCSW (Licensed Clinical Social Worker) or LPC (Licensed Professional Counselor) License are eligible to receive a $25,000 stipend for the 2025-2026 school year. Availability of this stipend in subsequent years will be determined based on future grant funding. 
  • 10,000 Bilingual Stipend: Candidates who successfully complete the district's bilingual proficiency assessment in Spanish and agree to accept additional Spanish-speaking clients as assigned will receive a $10,00 stipend. 

SUMMARY OF JOB DESCRIPTION:

The School-Based Clinician is responsible for providing immediate and ongoing (time-limited) clinical mental health services to students in need Primary function is to assess,triage, and provide evidenced based interventions for individuals who are seen as at risk by administration, or school staff and deemed to be in need of mental health services. The role of the school based clinician will be to ensure continuity of care: an immediate and professional evaluation, collaboration with proper agencies, referral to appropriate type and level of service, attendance at school meetings if deemed appropriate and advocacy for any treatment deemed necessary beyond initial crisis. The school based clinician will also conduct follow up individual or group sessions  with each child assessed and if on-going therapy is required, the clinician is able to provide that on school grounds at no charge to the student. 

 

SUPERVISED BY: School Based Therapy Program Supervisor

ESSENTIAL JOB ELEMENTS:

  • Has set school hours where presence is used for crisis assessments, individual or group therapy sessions, classroom interventions, or meetings 
  • Assesses the nature of any crisis and initiates appropriate response, intervention and level of service 
  • Where appropriate/necessary clinician will provide immediate crisis intervention services including suicide risk assessment, mental status exam and assessment of any other psychological and physical needs 
  • Collaborates with school, family or other appropriate persons, relevant to the situation at hand 
  • Staff cases, when necessary, with the SBTP Supervisor
  • Obtains proper paperwork and maintains strict adhesion to confidentiality 
  • Determines type and level of care for the individual assessed and documents accordingly 
  • Contacts other agencies (EMS, child protective services, law enforcement, etc.) when necessary 
  • Makes initial contact with any referring agency or private practitioner
  • Provides individual who was assessed (and/or their family) with necessary written information for follow up and education, also for any consent that may be done with the agency or therapist receiving the individual for treatment.
  • Conduct follow-up sessions with students to monitor progress and ensure ongoing support.
  • Initiates  individual and/or group therapy when appropriate and maintains a schedule to follow-up with  students on campus 
  • Maintains communication with school counselors on risk level or plan for treatment 
  • Maintains communication with other school personnel as needed on collaborative care for student 
  • Will be part of the ECSD and School specific crisis response team 
  • Attends all staff meetings and participates in weekly supervision with SBT team
  • Document all clinical records in the AHC medical records system, ensuring accuracy and compliance with agency and school policies and within the required timelines. 
  • Stay informed and adhere to all policies and procedures of both AHC and the assigned school(s) to ensure seamless integration of services.

Knowledge & Skills to:

  • Proficiency in DSM 5-TR diagnoses, symptoms, and treatment modalities.
  • Familiarity with local and agency resources for mental health referrals and wraparound services. 
  • Expertise in assessing suicidality, self-harm risks, and crisis intervention strategies. 
  • Ability to build rapport and work effectively with diverse populations, including students from various cultural, socioeconomic, and neurodiverse backgrounds.
  • Proficiency in basic computer operations, including Microsoft Word, electronic health records (EHR), and data management systems for documentation and reporting. 

Ability to:

  • Establish priorities, work independently, and proceed with objectives without direct supervision
